If you retire before you are age 65 (when eligible for Medicare) then you sign up whenever you want your monthly benefits to begin and then, at age 65, you are automatically enrolled.
If you wait until age 65 you can sign up for both Medicare AND monthly benefits OR you can just restrict your application to Medicare only and file later for monthly benefits when you decide to retire.
It's whatever you decide you want to do. And when you want to file you can do so online at ssa.gov or you can call 1-800-772-1213 and make an appointment to file either over the phone or in the office.
